Step 1

Call the office or fill out the attached form to schedule your 30 minute telehealth Initial Certification appointment. Once your appointment is scheduled we will need you to provide a picture of your Delaware driver's license or proof of residency (picture of utility bill, mortgage statement or rental agreement etc dated within the last 30 days).

Qualifying conditions recognized by the state of Delaware:
Adult Patient Qualifying Conditions

  1. Terminal illness
  2. Cancer
  3. Positive status for Human Immunodeficiency Virus (HIV Positive)
  4. Acquired Immune Deficiency Syndrome (AIDS)
  5. Decompensated cirrhosis
  6. Amyotrophic Lateral Sclerosis (ALS / Lou Gehrig’s Disease)
  7. Agitation of Alzheimer’s disease
  8. Post-traumatic Stress Disorder (PTSD)
  9. Intractable epilepsy
  10. Autism with self-injurious or aggressive behavior
  11. Glaucoma
  12. Chronic Debilitating Migraines
  13. A chronic or debilitating disease or medical condition or its treatment that produces one or more of the following
    1. Cachexia or wasting syndrome
    2. Severe, debilitating pain that has not responded to previously prescribed medication or surgical measures for more than three months, or for which other treatment options produced serious side effects.
    3. Intractable nausea
    4. Seizures
    5. Severe and persistent muscle spasms, including but not limited to those characteristic of Multiple Sclerosis

Step 2

After your appointment, complete your patient application at https://delaware.biotrackthc.net/patients/actions pay for your application ($50 paid to the state of Delaware). They are currently mailing cards within 2-4 weeks.

Step 2

After your appointment your certification will be emailed. Print and head to the dispensary of your choice. At no added cost, we can email your certification to a dispensary of your choice in case you forget your copy.

Step 2

Book your telehealth appointment with a licensed Maryland Medical Cannabis provider.  The telehealth will last approx 30 minutes for Initial Certifications or 15 minutes for certification renewals.  Your appointment is a comprehensive evaluation of your need for medical cannabis.  Once your telehealth appointment is complete and your paperwork is complete (we email BEFORE your appointment) your certification will be active and you can head to your local dispensary to purchase.  Your certification will last for 1 year, we will reach out as your renewal is approaching.

Step 1

Apply on the Maryland Medical Cannabis Commission website to get your patient ID.  It takes from 2 days to 2 weeks for the MMCC to email your patient ID number (it starts with the letter “P” so keep an eye out).  Your patient ID application costs $25 which is paid at the end of the application to the state.  Your number lasts 6 years.

Things you will need to complete your application:

  1. A valid, accessible email account.
  2. Full 9- digit Social Security Number
  3. An electronic copy of a valid government-issued photo ID (Driver’s license, State issued photo Identification card, Military ID (both front and back), US Passport)
  4. Proof that applicant lives in Maryland
  5. A passport style photo to upload to your application. The photo needs to be from your neck up with an all white background. If you would like help with the photo or have questions feel free to call or send us your picture for editing.

(All documents must be dated within the past 90 days)
